I, the parent/guardian, give permission for my child to participate in the Des Moines Panthers FC Kids Fun Run Program. I understand this is a recreational, non-competitive running program for children ages 4–11. I acknowledge that physical activity carries inherent risks and I have disclosed any relevant medical conditions. I agree to ensure my child is accompanied by a responsible adult. DMP Panthers FC coaches are not responsible for children outside of session time (5:00–5:30 PM). Participation is completely free of charge.
